Social Worker - MSW (Sickle Cell Disease Population) (Finance)



Nemours is seeking a Social Worker to join the team! This full-time, grant-funded social work position is dedicated to serving the pediatric sickle cell disease patient population in Sussex County, Delaware. This position is currently funded for two years with consideration of continued funding based on demonstrated patient/family need.

The social worker in this position will work collaboratively with the Nemours multidisciplinary hematology team assisting with patient sickle cell disease (SCD) clinic visits in our Milford and Wilmington locations.

Responsibilities will also include:

1) Assisting new patients and families with integration into the Nemours Children's Health SCD program

2) Providing education on disease management which reinforces clinical guidance given by SCD providers

3) Helping to track patients' routine care and supporting adherence to treatment via personalized follow-up/reminders

4) Working with the SCD clinical team to identify barriers to care and facilitating patient/family access to appropriate community resources

5) Conducting home and school visits when needed

6) Facilitating connection to language services

7) Hosting SCD community awareness events for patients and families. Bilingual Haitian Creole a plus.

Qualifications:

  • Master's degree in social work from a program accredited by the Council on Social Work Education (CSWE) required
  • Six months or more experience (Graduate school internship may be applied to the job-related experience at the discretion of the Senior Director)
