🧩 What You'll Be Doing:
- Coordinate and oversee accurate, timely MDS assessments and care plans
- Collaborate with the interdisciplinary team to ensure the highest quality of resident care
- Ensure documentation is thorough, compliant, and reflects the true picture of resident needs
- Help track quality indicators and support QAPI initiatives
- Be the go-to for MDS guidelines, updates, and coordination
🎯 What We're Looking For:
- Current LVN license in good standing (required)
- MDS experience preferred, or strong willingness to learn and grow in this role
- Strong understanding of RAI process and care planning
- Excellent communication, time management, and documentation skills
- A team-first attitude with a genuine passion for elder care
- Availability for a Monday-Friday schedule
