Occupational Therapist - Home Based Hospital Care (Finance)



Work Schedule :

50% FTE, Day shifts Monday through Friday. The position is eligible for up to a $2,000 sign-on bonus (pro-rated based on FTE). This is a hybrid role. When in person you will work at our Administrative Office Building in Madison, WI.

We are seeking an Occupational Therapist (OT) to join our Home-Based Hospital Care Team! You will:

  • Deliver acute, hospital-level care to patients in their homes using an evidence-based and patient-centered therapy approach.
  • Help patients regain independence, improve daily function, and safely navigate their home setting, reducing hospital stays and improving outcomes.
  • Work in collaboration with the interdisciplinary team to ensure that patients receive exemplary care in their home.

Qualifications

  • Bachelor's Degree in Occupational Therapy Required

Work Experience

  • 2 years prior relevant experience Preferred

Licenses & Certifications

  • Licensure as an Occupational Therapist in the State of Wisconsin Upon Hire Required
  • Basic Life Support (BLS) certification within 3 months Required
