COTA (Certified Occupational Therapist Assistant) - Sherwood Oaks Post Acute (Healthcare)

• Provide patient treatment as directed by the Occupational Therapist and per physician's plan of care.
• Document evaluations, daily treatment notes, weekly progress notes, and recertifications per OT Board and payer requirements.
• Communicate patient progress, issues, and plans with supervisor and healthcare team members.
• Assist in training Restorative Aides and instruct patients, families, and nursing staff on maintenance programs for discharge.
• Participate in in-service training programs and discharge planning activities.
• Maintain cleanliness and orderliness of treatment areas; report equipment issues promptly.
• Ensure compliance with state licensure, continuing education, and professional standards.
• Utilize computer programs (e.g., Casamba, Point Click Care) for documentation and data entry.
• Perform job duties requiring frequent standing, walking, lifting (up to 100 lbs occasionally), and other physical demands.
• Work effectively with patients, families, staff, and professional colleagues in a low-to-moderate noise environment.
